Life is crazy right now, so it took me a while to notice that the Google Form is outdated. And then it took me a bit more to finally bring it up to date. So, now, I have changed the link in the original post, will send it to the emails I have received and, in case your are reading this, you can simply head

to fill out the form. I will also share this on Instagram. Everyone needs to fill out this new form, but only needs to do this once.
